    what's the proper way to create a work request form for a web design company? I am creating one right now but I don't think it's professional enough. This is what I have:

    Basic Information (provided by client)

    Select Work Description (they can mark a check box for HTML, Flash, Photo editing, etc)

    Description Of Work To Be Performed:

    Completion Date:
