    The first question I have is ...
    Are you using PHP (or some server-side scripting) and a database (like MySQL)
    to get the data it needs for your form? Those forms are dynamically generated
    by a script ... where the drop-down lists are information from a database.

    The calendar part is usually a JQuery date-picker (javascripting).

    To process the form, requires PHP ... and as I asked ... a database?

    So, I would say that the form has two parts that need the database.
    You have to decide how to create your "From" drop-down selections,
    and the "To" drop-down selections. Are you going to "hard-code" all of
    the locations into those select boxes, or are you going to use your
    database to only show the destinations that your service provides?

    The other boxes are "open-ended". The user puts the data into those.
    A JQuery date-picker lets them pick a date. I like to use this one:

    Your first post said you needed "help" with the form. How much of the
    form have you currently done? I would say that you create the form how
    you want, and hard-code "from" and "to" destinations to begin with.
    When the form is how you want, replace the hard-coded <select> boxes
    with PHP scripting that uses the database to create those. Add the
    JQuery (javascripting) for the date-pickers, and show us your completed form.

    The form needs to be done before the PHP processing script.
    And since you have a MySQL database, I assume you've already done a lot
    of scripting ... and you're already using that database for other things?
