    There are other factors to consider, like captcha ... so spammer robots don't fill your email with spam.
    So the forms can be simple, no features, or it can be really complicated.

    Thanks Guys,

    I'm not sure if I have to worry about spammers as the website will only be visited by a relatively small amount of people, mostly only people I give my printed portfolio to.

    I looked at those links, I actually have no idea about PHP or how to use it. All the forms I've seen have so much info - I really just want a barebones one as I drew.

    Create your form (the HTML part) and we'll provide the PHP part.
    There are tutorials for HTML form examples.
