    I need a request a quote form for my website.

    There are numerous ways to do this...

    But you need to provide more info.

    Like:

    What scripting language does your website hosting package support ?

    Windows based servers typically use asp or asp.net, Linux based servers typically use PHP.

    And they don't interchange.

    #2 . What is the quote for ? Reason I asked, many companies these days don't and won't accept personal information submitted via email ? Why ? Because hackers tend to target those sites... So if you're asking for personally idnentifiable information ... You may want to setup SSL and save that data to a secured DB, but that in itself is another security issue as well.

    #3. What is your skill level ?
