    Welcome to WDF.

    The [b]creating the[/form] part is fairly straight forward.

    The difficult part comes in uploading the images. You need more than HTML knowledge to achieve that.

    You do not need to create a database, though it is a good idea so you can keep track of who is uploading what and so on.

    Your html form may look like:

    HTML Code:
    <form name="uploadfiles" action="/admin.php" method="post" enctype="multipart/form-data">
    Name: <input type="text" name="full_name" size="20" maxlength="100" />
    Comment: <textarea name="comment" rows="5" cols="30"></textarea>
    <p><input type="file" name="files[]" /></p>
    <p><input type="file" name="files[]" /></p>
    <p><input type="file" name="files[]" /></p>
    <p><input type="file" name="files[]" /></p>
    <p><input type="file" name="files[]" /></p>
    <input type="submit" name="submit" value="Upload" />
    Do you know any server side language?
