    I have cretaed a simple form in Dreamweaver and uploaded it to mywebsite. how to I go about adding a button that will collect the information and send it to me via email?

    You're going to need a server side language for the email part. For adding the button though it would be:

    Your page with the form needs to be online (on a website) ...

    The <form> line would look something like this ...

    <form action="sendmymail.php" method="post">

    Then, you would have a separate PHP script called "sendmymail.php" ...
    which would be the server-side script that actually sends the form to your email.

    That PHP script depends on whether you're sending a plain text email, an HTML
    email, an email with attachment, etc. There are hundreds of PHP form email
    scripts you can find using Google.

    Check this out the admin of this forum posted a tutorial, although there are thousands out there. You will have to check if your webhost/deal has php support.
