    This is a basic HTML form.

    The 'who is this for' part is a select box.

    Its all really basic stuff and the link below should get you started.

    For that you would need to php, that is a very long tutorial, especially for a beginner.

    This gives a pretty good overview, however it will probably be far easier to pay someone to do it for you.

    If you look at your example URL, you'll notice they are using a secure server ...
    HTTPS instead of HTTP. The server needs to be secure to handle information
    like credit cards, personal data, etc.

    You're talking about emailing the information, which then would not be secure.

    Better, you should store the information in a secure server (database) and
    email a link to access the information (with a username/password).
    So you're looking at,
    Secure Server
    PHP/MySQL
    A customer/member registration script.
    Credit Card Merchant w/processing scripts and keys

    Jason nailed this one ... you should hire a professional programmer to help you with this.
