    Could someone tell me how to get this form to work please!?!

    I need someone to write the stuff I need, and where and how I need to put it in my code!

    I see you're using GoDaddy, so you're using their "gdform.php" script?

    I'm thinking that the way you have it now, it should be working.
    GoDaddy only sends form emails to the email address that's set-up with the account.
    Did you go into GoDaddy's control panel and make sure an email address is defined?

    Go into GoDaddy (your account) and check it out ... verify the email address assigned,
    and they also have some examples of using "gdform.php".

    Well, if you're not good with PHP and making forms that point the content to your email, you could always use the website called JotForm. It has their own script, develops one with your username, and you determine the email to send the content to.
    Try it out. I use it sometimes when I am low on time.
