    Alright so I have a site for a client and they would like a member login area where if a student of theirs want to add some information about them such as:

    add a photo, provide their name, name of school, school class (e.g. freshman, sophomore etc.), height, weight, GPA, stats, official test scores (e.g. ACT/SAT), speed (40yd dash time), bench press max

    Basically something like the attached screenshot.. Any help is much appreciated. Thanks

    Before we go forward, are you comfortable/capable of using a database like MySQL to store and access the data? Or do you intend to just have the info emailed and not stored?

    I can do it both ways... I figured a database is a better way to go anyhow

    So this is not so much a login page, as a place where, after logging in, the student can add information to a profile.

    You will need a relational database to store the student's name. a unique ID, photo, school, class, height, weight, GPA, stats, official test scores, etc...

    I am no good at actually writing databases, but I do know this one will be fairly complex. You might have better luck if you posted to the Database Systems Help Forum.
