    Hi
    I have a design for a purchase page where people need to select one of three options (radio buttons) then click a continue button and be directed to the next page based on their selection - see example http://www.awakeninthenow.com/images/shipping.jpg. Can anyone tell me how to code a form to do this?

    those are called "radio buttons"....

    you create them all with the same name... and the one the user selects is sent to the form when it's posted...

    [SIZE=11px][COLOR=rgb(170, 0, 0)]<input type="radio" name="group2" value="Water"> Water<br>
    <input type="radio" name="group2" value="Beer"> Beer<br>
    <input type="radio" name="group2" value="Wine"> <br>[/COLOR]
    [/SIZE]

    [COLOR=rgb(0, 0, 0)][SIZE=11px]But this would be just 1 element of the form...[/SIZE][/COLOR]
