    Adding subscription fields on a template

    Hello everyone. I'm new here and am new with html/css. I think I came pretty far but I need some help. This is what I want to do.

    here is my site - http://i.imgur.com/zWddgjD.png

    Using Microsoft express 4

    Here is the code they told me to use to put a field onto website

    <!-- Begin MailChimp Signup Form -->
    <link href="//cdn-images.mailchimp.com/embedcode/classic-10_7.css" rel="stylesheet" type="text/css">
    <style type="text/css">
    #mc_embed_signup{background:#fff; clear:left; font:14px Helvetica,Arial,sans-serif; }
    /* Add your own MailChimp form style overrides in your site stylesheet or in this style block.
    We recommend moving this block and the preceding CSS link to the HEAD of your HTML file. */
    </style>
    <div id="mc_embed_signup">
    <form action="//usa.us14.list-manage.com/subscribe/post?u=bef8d2f7fc98ae1b68489458b&amp;id=3807d9f4f7 " method="post" id="mc-embedded-subscribe-form" name="mc-embedded-subscribe-form" class="validate" target="_blank" novalidate>
    <div id="mc_embed_signup_scroll">
    <h2>Subscribe to our mailing list</h2>
    <div class="indicates-required"><span class="asterisk">*</span> indicates required</div>
    <div class="mc-field-group">
    <label for="mce-EMAIL">Email Address <span class="asterisk">*</span>
    </label>
    <input type="email" value="" name="EMAIL" class="required email" id="mce-EMAIL">
    </div>
    <div class="mc-field-group">
    <label for="mce-FNAME">First Name </label>
    <input type="text" value="" name="FNAME" class="" id="mce-FNAME">
    </div>
    <div class="mc-field-group">
    <label for="mce-LNAME">Last Name </label>
    <input type="text" value="" name="LNAME" class="" id="mce-LNAME">
    </div>
    <div class="mc-field-group">
    <label for="mce-MMERGE3">Website </label>
    <input type="url" value="" name="MMERGE3" class=" url" id="mce-MMERGE3">
    </div>
    <div id="mce-responses" class="clear">
    <div class="response" id="mce-error-response" style="display:none"></div>
    <div class="response" id="mce-success-response" style="display:none"></div>
    </div> <!-- real people should not fill this in and expect good things - do not remove this or risk form bot signups-->
    <div style="position: absolute; left: -5000px;" aria-hidden="true"><input type="text" name="b_bef8d2f7fc98ae1b68489458b_3807d9f4f7" tabindex="-1" value=""></div>
    <div class="clear"><input type="submit" value="Subscribe" name="subscribe" id="mc-embedded-subscribe" class="button"></div>
    </div>
    </form>
    </div>
    <script type='text/javascript' src='//s3.amazonaws.com/downloads.mailchimp.com/js/mc-validate.js'></script><script type='text/javascript'>(function($) {window.fnames = new Array(); window.ftypes = new Array();fnames[0]='EMAIL';ftypes[0]='email';fnames[1]='FNAME';ftypes[1]='text';fnames[2]='LNAME';ftypes[2]='text';fnames[3]='MMERGE3';ftypes[3]='url';}(jQuery));var $mcj = jQuery.noConflict(true);</script>
    <!--End mc_embed_signup-->
    field looks like this:

    However, what I want to do is leave the current field from the template and just use the functions in the code they gave me. I hope that makes sense?

    The template field code is this:

    Enter your details below</h2>
    <div style="background:url(images/bg_field.png) no-repeat; width:680px; height:45px; padding:20px 4px 20px 4px;">
    <ul style="list-style-type:none;">
    <li style="padding:0px 5px 0px 5px; float:left;"><input type="text" style=" padding:8px; background: url(images/bg_textfield.png) no-repeat; width:210px; font:15px Calibri; color:#999; border:none;" onclick="if(this.value=='enter your full name') this.value=''" value="enter your full name"></li>
    <li style="padding:0px 5px 0px 5px; float:left;"><input type="text" style=" padding:8px; background: url(images/bg_textfield.png) no-repeat; width:210px; font:15px Calibri; color:#999; border:none;" onclick="if(this.value=='enter your email address') this.value=''" value="enter your email address"></li>
    <li style="padding:0px 5px 0px 5px; float:left;"><a href=""><img src="http://www.webdesignforums.net/forum/images/access_btn.png" alt="access button" /></a></li>
    </ul>
    </div>
    <div style=" background:url(images/lock.png) no-repeat; padding:8px 0px 8px 0px; margin-left:30px; "><span style="font:12px Calibri; color:#000; padding-left:37px; text-align:left;">I respect your privacy and have a Zero Tolerance for spam.
    Your information is safe and secure.</span></div>
    </div>
    Basically just the design but doesn't have functions/doesn't link anywhere. Again, I want to add the functions from the code they gave me to the field already provided on the template. Thing is, I have no idea how. Also, the code provided is for 4 fields I think when the template only has 2

    I have no knowledge on css and I've mostly relied on google to find what I need but this one I can't get help on.
