    I think you are getting the cart before the horse. Newspaper headings came from a concept idea. HTML headers come from this same conceptual idea. HTML headers do not come from newspaper headers. Why should a heading be boldface rather than italic? What happens in scripts that don't lend themselves to well to bold typefaces, such as Japanese? It doesn't really make sense to say that the blank line is important.
    You have made a good point (though it contradicts your other points). "h1 ... should be easily recognised as such". And that's the main point. Yes if you set something as a header and it isn't recognisable in type as one, then that is simply bad design. But you can't make the inference that the only way to make a good header is through the default settings of white space, bold etc. A good header could be colored, float left, vertical for example.
