    Hi,

    I've created a form with php and I cannot see why I am not receiving the form submissions!!!
    I've used this form with other websites and its worked but not with this one!

    Please see the attached filed (both process.php and form.htm ) do you see anything wrong with the coding that may be the problem???

    As mentioned, when I submit the form - I never receive the email. Please help me with this, thanks!

    We are using a dedicated server from Godaddy.com ...

    You say your script works on other webhosts, but not Godaddy.
    That sounds like a Godaddy issue to me.

    Go into Godaddy and look for some PHP email examples.
    It's possible they don't allow the PHP mail() function ...
    or they require you to use a certain script that they have designed.
