I need for a user to enter a date in a form and would like to have a calendar pop up like I see on a lot of web sites. What is the best way to create this? I am using php and MySQL.

Use the free JQuery date picker ...

    See explanation and demos here:

This is a front end thing. No PHP or MySQL necessary (apart from the storage of the date).

So my pick for calendar selectors is to use jQuery UI which is a library for jQuery which has some styling with it as well. It has a built in datepicker plugin which you can view here:

    http://jqueryui.com/demos/datepicker/

Nice and easy to use! Then you you can convert it to a unix timestamp and store it, or convert it to Y-m-d 00:00:00 and that'l work too depending on your MySQL field type.
