So you created the files using DW ... and you uploaded them to your shared webhost (website)?
Or are you doing all of this on your own local server?

And you did all of the necessary PHP scripting and MySQL database creation?

So you have your own local Apache website server (like WAMP), with PHP 5+, and MySQL? is that correct?

You'll have to start with some tutorials ... like this:
    http://www.tizag.com/mysqlTutorial/mysqlconnection.php

Learn how to use PHP to connect to your database.

Make test scripts like the ones they have, but use your own
database name, table name, and column names.

See if you can connect and INSERT a row in your table.

Do the tutorials one step at a time.

^ Yeah that's the exact one I used when I started. I'd reccomend after getting to grips with those basics it'd be beneficial to look into PDO for your database access as the "prepare" statement it uses goes a long way to preventing SQL injection.

Just something to get your started!
