Problem with PHP On WordPress Site

I have a webpage that is being edited using WordPress. On the pages being posted is a form that allows the viewer to submit information to an email address. The problem I am having is the PHP file doesn't seem to be reading in WordPress, but the form works for a standard website. When attempting to submit the form, I receive an error that states:Not Found

The requested URL /index.php was not found on this server.

There is a file named index.php that does reside on the server. Is there something in the coding that I am missing, so the PHP page can be found? I was thinking a series of ../ in the action="process.php" tag would work, but it didn't. Any ideas?

To do forms yourself, manually, requires special WordPress scripting in your stand-alone PHP script ... the script that processes your form.

I'll save you a ton of grief ...

    Install this plugin:
    https://wordpress.org/plugins/contact-form-7/

Makes it easy to put forms into WordPress.

There was a problem with the contact form. I can apply it to the page, but I can't apply it to each individual post, which is a custom PHP page. Is there a way to add a form to each post that doesn't require plug-ins?
